Subject Access Request (SAR)

You have the right to apply to access any personal data that an organisation holds about you. You also have the right to give authorisation for someone else to make the application on your behalf. This type of request is known as a Subject Access Request or SAR.

Records NHS Shropshire, Telford and Wrekin Integrated Care Board  cannot provide

NHS Shropshire, Telford and Wrekin Integrated Care Board (NHS STW ICB) has limited access to patient information and can only provide data where we are the data controller.   If an individual is looking to obtain records from another service, such as a hospital, GP practice or community service, these requests will need to be made directly to the service.

How do I submit a Subject Access Request?

As noted above, NHS STW ICB holds limited patient records, there is a list of local providers below, with links to the relevant pages of their websites.  Please contact providers directly, as NHS STW ICB do not hold or have access to these records (for ease of reference links to individual organisations websites are included below):

The Shrewsbury and Telford Hospital Trust  (Including the Royal Shrewsbury Hospital and the Princess Royal Hospital, Telford)

Midlands Partnership University Foundation Trust  (provider of local mental health services, including the Children and Adolescent Mental Health Service (CAMHS))

Shropshire Community NHS Trust  (provides services such as community nurses, musculoskeletal services, community hospitals, community hospitals, etc)

West Midlands Ambulance Service (provides emergency patient transport)

NHS Shropshire, Telford and Wrekin Integrated Care Board (as the commissioner of services NHS STW ICB have limited teams that hold patient data, but are responsible for the Continuing Health Care assessment process, NHS STW complaints and PALS, individual funding requests and the referral management service)

GP Practices – Please search on individual GP Practices websites or call the practice to ask for the process for requesting copies of records.  NHS Shropshire, Telford and Wrekin ICB are unable to provide copies of GP records.
