Child Death Overview Panel
The first week of June is Child Safety Week, and the Child Accident Prevention Trust (CAPT) is making prevention possible by offering free resources to prevent devastating accidents.
CAPT has a comprehensive library of resources, freely available to support teams and avoid any duplication of effort. The charity also offers a wide range of safety advice.
You can equip local public health and community contacts with tools to engage families before tragedy strikes. Or find resources for your own use during the review process.
There are resources for display and to give to parents - please also share these to relevant professionals in your own networks.
Parents' pack – top tips to share with families. Includes all CAPT’s fact sheets on the most serious accident risks in one place.
Display pack – all that’s needed for a colourful, engaging display covering essential prevention messages.

The NHS Shropshire Telford and Wrekin Child Death Overview Panel (CDOP) reviews the deaths of all children between birth and the day before their 18th birthday.
This is a statutory requirement in accordance with the Child Death Review Statutory and Operational Guidance England (2018), Working Together to Safeguard Children (2023) and The Children Act (2004).
The Shropshire Telford and Wrekin CDOP Strategic Partnership Governance structure is shown below:

The Child Death Overview Panel (CDOP) review the deaths of all children in Shropshire Telford and Wrekin aged between birth and the day before their 18th birthday, this has been a legal requirement since 1st April 2008. This does not include stillbirths, late foetal loss and legal terminations of pregnancy.
The aim of this process is to ensure we learn from deaths and can put in place actions and initiatives in order to reduce the risk of children dying in the future.
We are also dedicated to ensuring our families have a voice and are signposted to support services/organisations which can provide them with bereavement support.
