Hospital Discharge

Help us get your loved ones home from hospital sooner.

When someone you care about is in hospital, we know you want them home as soon as they’re ready. This is especially the case during the festive and winter period. Home is where recovery happens best, surrounded by familiar faces and comforts. 

This winter, we need your help to make this happen. By supporting your loved one to leave hospital promptly once they’re ready, you’re helping them recover and freeing up beds for others who need urgent care.

How you can help to get your loved one home 

  1. Keep in Touch
    Stay connected with the hospital team and check when your loved one will be ready to leave. Knowing the plan helps everything go smoothly.
  2. Get Their Home Ready
    Before collection:
    • Switch on heating and check electricity and water. 
    • Tidy up and remove any slip or trip hazards. 
    • Stock up on fresh food and drinks for recovery, think balanced meals with carbs, protein, dairy, and fruit and veg. 
  1. Make Sure They’re Comfortable
    Bring clean, warm clothes for the journey home. Ensure they have access to a phone and money for essentials.
  2. Collect Them Quickly
    When the hospital says they’re ready, please pick them up as soon as possible. If you don’t have transport, consider a taxi or volunteer driver service. Ask the hospital if you need help arranging this.
  3. Share Support Information
    Once home, explain what help is available in the community. You may have leaflets from the hospital.
