GP Survey - Discharge Letters
SATH Patient Safety and Quality Improvement Teams are undertaking a project looking at hospital discharge letters which they acknowledge are an important handover document when transitioning care from hospital to general practice.
They would like to hear from Primary Care so they can capture your thoughts on the current processes.
They would be grateful if you could complete this survey to assist work to improve discharge letters.
