Care Transfer Hub: Helping Patients Return Home Safely and Confidently

The Care Transfer Hub at the Shrewsbury and Telford Hospital NHS Trust is helping patients return home safely, confidently, and with the right support in place. From the moment a patient is admitted, the Hub’s team of nurses, therapists, social workers, and support staff work together to understand each person’s needs and plan a discharge that is safe and tailored to them. 

By bringing together expertise from Shropshire Community Health NHS Trust, Shrewsbury and Telford Hospital NHS Trust, Powys Health Board, Shropshire Council, Telford and Wrekin Council, Shropshire Partners in Care, and local community and voluntary services, the Hub ensures that care is coordinated across organisations. This collaboration means patients experience a smoother, quicker, and less stressful journey from hospital to home. 

Central to the Hub’s work is the “Home First” approach. From the moment a patient is admitted, the team evaluates their care needs, aiming to discharge them safely to their own home wherever possible. Daily multidisciplinary ward meetings help plan estimated discharge dates and ensure that every patient receives tailored support. Therapy assessments, equipment delivery, and follow-up visits are coordinated by the Hub to make recovery at home as smooth as possible. Social workers arrange packages of care and ensure ongoing support, while pharmacy teams answer questions about new medications to help patients manage their health confidently.
