Shropshire Telford and Wrekin Palliative and End of Life Care Strategy (Adults) 2022 – 2025

This strategy has been developed because of recommendations from a review of Palliative and End of Life Care that was completed at the end of 2021. 

Over the period of the review heath and care colleagues, commissioners, and people with lived experience of caring for a loved one at the end of life worked together to identify what currently works well and what needed to be improved. Key recommendations following this work have been captured in this strategy. 

To enable organisations and services to work better together it was necessary to have a collective understanding of how, as a health and care system, we could ensure that people in the last year of life would have access to the right care at the right time in the right place.  

This strategy is one of the steps we have taken to ensure that Palliative and End of Life Care is seen as a priority for Shropshire Telford and Wrekin Integrated Care System. Over the next three years we will continue to work together to implement this strategy and create the mechanisms needed to measure the positive impact that we are expecting to see.
