Transforming Care Programme Service Transfer

From 1 August 2026, the Transforming Care Programme (TCP) service will transfer from Midlands and Lancashire Commissioning Support Unit (MLCSU) to North Staffordshire Combined Healthcare NHS Trust (NSCHT). This change is taking place because commissioning support units are closing nationally and alternative arrangements are needed to ensure the continued delivery of the service locally.

What is the Transforming Care Programme (TCP)

TCP focuses on improving health and care services for individuals with learning disabilities and autistic people. Key aims include:

  • Reducing hospital admissions for individuals with complex needs and enhancing care outcomes. 
  • Integrating services from health, social care, and education to ensure better support for local communities. 
  • Implementing case management to provide robust support mechanisms and improve patient experiences. 

  • Collaborating with local authorities and healthcare providers to ensure coordinated care pathways.

This program is designed to help individuals live safely and comfortably in their communities, rather than in hospitals. 

What does the transfer mean for people who use the service?

We do not expect any disruption to the support people currently receive. Existing referrals, assessments and care arrangements will continue, and the service will remain commissioned by NHS Shropshire, Telford and Wrekin ICB. The staff who currently deliver the service are expected to transfer to NSCHT, helping to maintain continuity of support.

Do I need to do anything?

No. You do not need to take any action unless you are contacted directly.

Information for professionals and partner organisations

Referral pathways will remain in place during the transition period and the aim is to ensure a seamless transfer with no disruption to people receiving support. NHS Shropshire, Telford and Wrekin ICB, MLCSU and NSCHT are working closely together to ensure a smooth transition and maintain strong working relationships with local partners.
